Appeal Form

Pre-Certification appeals must be filed within 30 days after the application decision or the release of the CP^® Exam results. Certification appeals must be filed within 30 days after the expiration date of the certification. CLE Extension requests for hardships or extenuating circumstances must be submitted at least 30 days prior to the end of the certification period. Appeals may be mailed, faxed, or emailed to cle@nala.org.
